This workshop is facilitated by Raechel Callejo, Licensed Marriage and Family Therapist #119641, licensed in the state of California. The workshop is offered through North State Family Counseling Center, Inc., a California Professional Corporation, of which Raechel Callejo, LMFT is the CEO. The business address is 410 Hemsted Dr., Suite 110, Redding, CA 96002. The purpose of this workshop is psychoeducational in nature and is not intended to replace individual or couples therapy. Participation does not establish a therapist-client relationship with Raechel Callejo, LMFT or North State Family Counseling Center, Inc. While the workshop is designed to promote personal insight and relational growth, outcomes will vary, and no specific results are guaranteed. This program is not suitable for couples experiencing active or undisclosed affairs, serious domestic violence, or untreated severe mental health conditions such as schizophrenia or active suicidality. If any of these apply, participants are asked to contact Raechel Callejo, LMFT at rcallejo@northstatecounseling.com to discuss cancellation and explore alternative support options if appropriate. Confidentiality is expected within the group setting; however, due to the nature of group participation, it cannot be guaranteed. Group members are expected to keep all information they learn about others confidential. All facilitators will uphold HIPAA standards and are legally required to report suspected abuse of children, elders, or dependent adults, as well as any credible threats of harm to self or others. Participation in group discussion is voluntary but highly encouraged, as engagement often enhances learning and overall benefit. By registering for this workshop, participants acknowledge and accept the terms outlined above. Refund & Cancellation Policy Please note that no refunds will be issued to participants who register without reading this form and are later determined to be inappropriate for the workshop due to the exclusion criteria outlined above. A full refund (minus a $50 processing fee) will be granted if cancellation is requested at least one full week prior to the workshop start date. Alternatively, the full amount may be applied to a future workshop within one year. Cancellations made less than one week in advance are not eligible for a refund but may still be credited toward a future workshop within one year. No refunds will be issued for no-shows.
